The policy must provide at least 3,000,000 Thai Baht of in-patient care coverage and must cover the entire 12-month duration of the visa validity period. Travel insurance and life insurance are not accepted.

As part of the OA application, you must provide a medical certificate signed by a licensed doctor confirming that you are free of the following five conditions: Leprosy, Tuberculosis (T.B.), Elephantiasis, Drug Addiction, and Third Stage of Syphilis. The certificate must be no older than 3 months at the time of application.

This is typically completed using the Thai government's prescribed medical certificate form. Most applicants simply print the form and bring it to their regular family doctor or GP to complete and sign.
