Is health insurance actually required for the Non-O (vs the OA), and is a medical certificate needed?

No on both. The Non-O retirement visa doesn't require health insurance or a medical certificate. Those are OA requirements: the OA needs 3,000,000 THB of inpatient insurance, a medical certificate, and a police clearance. That insurance-and-paperwork difference is the main reason the OA is more expensive and more involved than the O.
