What medical certificate is required, and how do I get it?

As part of the OA application, you must provide a medical certificate signed by a licensed doctor confirming that you are free of the following five conditions: Leprosy, Tuberculosis (T.B.), Elephantiasis, Drug Addiction, and Third Stage of Syphilis. The certificate must be no older than 3 months at the time of application.

This is typically completed using the Thai government’s prescribed medical certificate form. Most applicants simply print the form and bring it to their regular family doctor or GP to complete and sign. It is a straightforward process for most people, and in the majority of cases a standard family physician appointment is all that is needed.

One critical constraint: the doctor who signs the certificate must be licensed and located in the same country where you are submitting your OA application. If you are applying through the Thai Embassy in the US, the doctor must be in the US. If you are applying through the Embassy in Canada, the doctor must be in Canada. A certificate signed by a doctor in a different country from where you are applying will not be accepted.
